Don't know if you're eligible? Take the test. Your results may suprise you.
If you score a moderate or high risk, or if you already have cardiovascular disease, you could be eligible to receive free one on one health coaching services from the Ottawa Heart Institute. The platform accepts men and women of all ages.
Please note: You MUST be a resident of the Champlain District to receive health coaching. A health coach can help guide and educate you on how to better manage your risk factors for cardiovascular disease.
